What Is Peroneal Nerve Dysfunction?

The common peroneal nerve (also known as the common fibular nerve) is one of the two main nerves that branch off the sciatic nerve. The sciatic nerve is the largest and longest nerve in the body – it starts from the lower back and is responsible for innervating the body’s lower extremities. 

The peroneal nerve, which starts at the top of the glutes, has a similar, but more specific, function. It travels down the back of the thigh and wraps around the knee, where it splits into two main branches:

  • A deep peroneal nerve runs on the inside of the leg and over the ankle bone. It is responsible for controlling the function of areas of the foot, such as the big and second toes. 
  • The superficial peroneal nerve runs on the outer aspect of the lower leg, and it provides sensation and movement control to the top of the foot and the other toes.

It is thanks to the peroneal nerve that you are able to experience pain, temperature, and touch sensations throughout your legs. This nerve also plays a major role in enabling movements such as lifting your toes and ankle. 

Peroneal nerve dysfunction may occur when this important nerve is compressed, damaged, or irritated. This can occur due to surgical interventions that affect the knee (e.g. knee replacement surgery), traumatic injuries like fractures and dislocations, and conditions that damage the nerves, such as diabetes. 

Mild peroneal nerve injuries can lead to numbness, tingling, the feeling of pins and needles, weakness, and foot pain, while more severe nerve injuries can cause mobility problems, disability, and complications like foot drop. Foot drop is defined as an abnormal walking pattern (gait) in which you are unable to clear your foot off the ground and drag your toes due to the inability to flex the ankle and lift the foot upward. 

Although anyone can develop a peroneal nerve injury, this disorder is more common if you have inflammatory conditions like rheumatoid arthritis, Type 2 diabetes, or anorexia nervosa. Traumatic injury to the leg (such as fractures and dislocations), as well as habits such as crossing your legs when you sit, can also lead to this condition.
